1.J820 is only eligible for payment for workup of hyperparathyroidism with biochemical evidence (e.g., elevated serum PTH) or inappropriately normal serum PTH elevated normalized serum calcium or ionized calcium.
2.J820 is not eligible for payment for evaluation of an incidental nodule found on anatomic imaging (e.g. ultrasound, CT, MRI) in the absence of a biochemical abnormality.
